So let's start with the string being the most obvious part as it is. So, the string represents a line to the heart, commonly associated with a bond of love. The string is strong that's why it appears as though there are two connected to her going to the same source. The bond is so strong in fact that it is pulling her away from where she used to be and into a bright new future. The place she is coming from was a dark and harsh place (represented by the strong shadows cast behind her) and the place she is going (apparently towards her love) is a much better friendlier place (represented by the light and the hopeful upward tilt of her head. To survive in the dark she lived in she had to become a monster (as shown by the horns on her head) just to survive. She is not out of the shadows yet though as can be seen be the incomplete figure (representing the partial transition into the light) and the horns that remain on her head. The blood could represent something she had to sacrifice in order to follow her heart. Alternating it could be from an old wound caused by a similar bond that once hurt her very badly, enough to leave a lasting wound.
Of course this could be completely meaningless and something the artist drew just because they could.
